I think this is a great article. But there are two strong impressions I'm left with: One, all of these mistakes can be and often are made during the course of "normal operations" - they're just more likely to go unnoticed. Two, there is a lot of wisdom in the old adage about people making their own luck . . .

I'm currently doing my doctorate in sustainable change and one of the biggest issues to overcome is how to maintain behaviour in times of crises. For all the research that has been done in this area no one seems to have cracked the issue that when under stress people revert back to type, the only area that seems to have resolved this is the military (to a degree) where they practise and train to the point of instinctive behaviour - I can't imagine that ever happening in a board room !!
